What actually causes falls from height

Understanding the causes helps you take preventive action today. These factors drive most falls in UK workplaces.

Unplanned work

Quick jobs done without planning are where falls cluster. A two minute task at height deserves the same thought as a two day one.

Wrong equipment

A ladder used where a podium or tower was needed, or a pallet on forks used as a platform. Equipment shortcuts cause falls.

Unprotected edges

Open edges, floor openings and stairwells without guardrails are falls waiting to happen, especially in changing work areas.

Fragile surfaces

Roof lights, cement sheets and corroded roofing give way without warning. Fragile surfaces must be identified before anyone steps near them.

Weather and conditions

Wind, rain, ice and poor light all raise fall risk. Work at height should be rescheduled when conditions make it unsafe.

Poor maintenance

Equipment that is not inspected - damaged ladders, worn harnesses, missing guardrail components - fails when it is needed most.

WAHR 2005 sets the order of decisions for every task at height. Start at the top and only move down when the step above is not reasonably practicable.

Avoid work at height

The safest option is not to go up at all. Use extendable tools from the ground, lower equipment for servicing, or assemble at ground level and lift into place.

Use an existing safe place

Where height is unavoidable, work from a place that is already safe - a permanent platform with guardrails, a flat roof with parapets, or a fixed walkway.

Prevent falls with collective protection

Guardrails, toe boards and barriers protect everyone in the area without relying on individual behaviour. Collective measures always come before personal ones.

Choose equipment that prevents the fall: working platforms, tower scaffolds, MEWPs and podium steps with guardrails built in.

Fall arrest: the safety net, not the plan.

Fall arrest does not prevent a fall. It catches one that has already started, which means the fall, the forces and the suspension all still happen. A fall arrest system is only acceptable when avoiding the work and preventing the fall are not reasonably practicable.

Used properly, it needs a suitable anchor point, a correctly fitted harness, an energy absorbing lanyard, enough clearance below, and a rescue plan that gets a suspended person down quickly. Practical harness training may be legally required for these tasks and is not replaced by an online course - our training makes that distinction clear.

How the hierarchy of control works in practice

The hierarchy is a sequence of questions asked in order, for every task. Can we avoid going up at all? If not, can the person work from a place that is already safe? If not, can we prevent the fall with guardrails, platforms or restraint? Only when the answer to all of those is no does minimising the consequences - nets, soft landing systems and finally fall arrest - come into play.

Avoid: the step most often skipped

  • Extendable tools - poles for cleaning, painting and inspection keep feet on the ground.
  • Ground level assembly - build at ground level and lift into position with appropriate plant.
  • Relocating equipment - move valves, filters and controls to accessible positions during design or refit.
  • Cameras and drones - inspect roofs and structures without putting a person up there.

The most effective fall prevention measure in the world is the task you redesign so nobody has to leave the ground.

Prevent: guardrails, platforms and restraint

Where work at height cannot be avoided, prevention measures stop the fall from happening:

  1. Guardrails and edge protection - a top rail, mid rail and toe board around every open edge and opening, installed before work begins and inspected regularly.
  2. Working platforms - scaffold platforms, tower scaffolds and MEWP baskets that give a stable surface with protection built in.
  3. Podium steps - a guarded platform for low level work that removes the open sides of a stepladder.
  4. Covers and barriers - secured, labelled covers over floor openings and barriers keeping people away from fragile surfaces.
  5. Work restraint - a harness and fixed length lanyard that physically stops the worker reaching the edge.

Minimise: when some risk remains

Where prevention is not reasonably practicable, the duty becomes minimising the distance and consequences of a fall. Safety nets and soft landing systems installed close under the working level catch a fall after centimetres rather than metres. Personal fall arrest - the harness, energy absorbing lanyard and anchor - is the final layer, and it only works as a system: anchor strength, clearance below, pre-use checks, trained users and a rehearsed rescue plan all have to be in place before the first foot leaves the ground.

Protecting people below the work

Fall prevention includes objects, not just people. Toe boards stop tools sliding off platforms, tool lanyards tether kit to the user, debris netting catches small items, and exclusion zones keep everyone clear of the drop zone. A spanner from height injures just as surely as a fall.

What is the hierarchy of control for work at height?
WAHR 2005 sets a clear order of decisions: avoid work at height where reasonably practicable, prevent falls where it cannot be avoided, and minimise the distance and consequences of a fall where the risk remains. You work down the hierarchy only when the step above is not reasonably practicable.
Why are guardrails better than a harness?
Guardrails are collective protection. They protect everyone in the area without relying on individual behaviour, training in harness use, or a rescue plan. A harness is personal protection that only protects the wearer, only works if it is fitted and anchored correctly, and still allows a fall to happen in arrest systems. That is why collective measures always come first.
When is fall arrest acceptable?
Fall arrest is a last resort, used only when avoiding the work and preventing the fall are not reasonably practicable. It must be backed by a suitable anchor point, a properly fitted harness, adequate clearance below, trained users and a rescue plan. Without a rescue plan, fall arrest is not a complete system.
What counts as a working platform?
Any surface used as a place of work at height, including scaffold platforms, tower scaffolds, MEWP baskets and podium steps. A compliant working platform has guardrails, toe boards, a suitable surface and enough strength for the people and materials on it.
Do I need to protect people below the work area?
Yes. Falling tools and materials injure people just as falls do. Use toe boards, tool lanyards, debris netting and exclusion zones to keep people clear of the area beneath the work.
Is fall prevention training a legal requirement?
